A comprehensive, up-to-date medication list is a caregiver's most vital tool. It must include all prescriptions, over-the-counter drugs, dosages, and prescribing doctors. Keep digital copies on your phone and a physical copy on the senior's refrigerator for emergency responders.
In a medical emergency, paramedics and emergency room doctors will immediately ask: "What medications are they taking?" In the panic of the moment, trying to remember the names and dosages of half a dozen complex drugs is nearly impossible. Having a prepared, accurate Medication Master List is quite literally a lifesaver.
What to Include on the List
A proper medication list goes far beyond just the names of the drugs. Your master record should be a spreadsheet or clearly written document that includes:
1. Medication Name: Both the brand name and the generic name.
2. Dosage & Frequency: Exactly how much is taken and when (e.g., 20mg, twice daily).
3. Purpose: Why they take it (e.g., for high blood pressure, for nerve pain).
4. Prescribing Doctor: The name and contact info of the physician who ordered it.
5. Allergies: Any known drug allergies prominently displayed at the top.
6. OTC & Supplements: Do not forget to list daily vitamins, herbal supplements, and over-the-counter pain relievers, as these can interact with prescriptions.
Where to Keep It
You need redundancy. Keep a digital copy in the "Notes" app of your smartphone so you have it at every doctor's appointment. Keep a printed copy folded in the senior's wallet. Most importantly, tape a highly visible copy to the senior's refrigerator. EMTs are trained to look on the fridge for medical information when responding to an elderly patient's home.
The "Brown Bag" Review
Whenever you visit a new specialist or the primary care doctor for an annual checkup, bring the Master List, but also bring the "brown bag." Literally put every single pill bottle the senior uses into a bag and bring it to the appointment. This allows the doctor to verify exactly what is being taken, check expiration dates, and update your records accurately.
