How to Properly Thicken Liquids for Seniors with Swallowing Issues

Thickened liquids are vital for seniors with dysphagia, as thin liquids move too quickly and can enter the lungs. By understanding the different thickness levels (nectar, honey, pudding) and using commercial thickeners correctly, caregivers can ensure safe and adequate hydration.

Hydration is a constant challenge in elderly care, but it becomes significantly more complex when a senior has dysphagia. Thin liquids like water, juice, or coffee move very quickly in the mouth. For a senior with delayed swallowing reflexes, these liquids can easily slip into the airway before the swallow is triggered, leading to choking or aspiration pneumonia. The solution? Thickened liquids.

 Thickening a liquid slows its flow, giving the throat muscles more time to control the swallow. However, thickening must be done correctly according to a Speech-Language Pathologist’s (SLP) recommendations. The International Diet Standardisation Initiative (IDDSI) categorizes liquid thickness into specific levels:

Slightly Thick: Thicker than water but can still flow through a standard straw.

Mildly Thick (Nectar-thick): Pours quickly but leaves a thin coating on the cup. It flows off a spoon in a steady stream.

Moderately Thick (Honey-thick): Pours slowly and coats a spoon thickly. It requires a bit of effort to drink from a cup.

Extremely Thick (Pudding-thick): Holds its shape and must be eaten with a spoon.

 How to Thicken Liquids:

1. Choose the Right Thickener: Commercial thickeners come in starch-based or xanthan gum-based powders. Xanthan gum is often preferred as it doesn't alter the taste and remains stable over time.

2. Measure Accurately: Always follow the manufacturer’s instructions. Use the exact scoop provided and measure the liquid precisely.

3. Stir Vigorously: Add the powder to the liquid while stirring briskly with a fork or whisk to prevent lumps.

4. Let it Rest: Most thickeners need 1 to 5 minutes to reach their final consistency. Do not serve immediately.

 Remember, thickened liquids can feel unappealing at first. Encourage your loved one by serving their favorite flavors and ensuring beverages are served at the proper temperature. Proper hydration is essential, and with patience, thickened liquids can become a seamless part of their routine.
